We welcome new patients and thank you for the opportunity to serve your dental needs. The information that follows is designed to answer many questions new patients may have. We want you to be aware of our policies and methods of practice. We are dedicated to providing you with the highest quality of dental care possible in a relaxed, stress-free office.
Dentistry Today:
Dentistry today is very different than just a few years ago. Cosmetic Dentistry has become very popular. There are many things we can do to give patients a nicer smile. If you have concerns about the appearance of you teeth please ask us.
Insurance Claims:
We will submit our fees to your insurance company as a courtesy. It is in your best interest to be aware of the coverage your policy has and it's limitations and exclusions.
Patient Payment Guidelines:
All patients are requested to pay at the time of service for all first time services. We request payment of co-payments and deductibles at the time of service. All patients are encouraged to discuss fees BEFORE a procedure is performed. The fee is your financial responsibility. We do not recognize your insurance companies Usual & Customary fee as payments in full. The insurance company's fees allowed are determined by the premium paid into, not by our fees. The more the premium the more the coverage. Less expensive premiums, less coverage.
Preferred Providers, HMO's etc:
I have chosen not to sign any contracts with insurance companies. A contract dentist is first responsible to the insurance company and secondly to the patient. I feel that this is backwards. The dentist should be responsible to the patient first...so keeping with that philosophy I have not signed contracts with insurance companies, You will not find my name on their lists.
Extended Payment Plans:
We do accept Master Card, Visa, and Discover for payment of services. We do however have extended payment plans offered through General Finance of Green Bay. We also offer a 90 day payment plan at no interest for qualified patients.
Appointments:
All patients are seen on an appointment bases. We do see our patients of record on a same day basis for emergency care.

Cancellation Notice:
We ask for a minimum of 24 hours notice if you must change an appointment. This allows us to see patients who are waiting to have treatment done sooner or for a patient with an emergency problem to be seen. Occasionally an emergency can cause problems/delays in our appointment schedule, We know people hate to wait, are busy and have better places to be than a dental office so we really try to be very efficient and to treat patients promptly.
